3. Performance rating in the present position for one (1) semester (if applicable);
4. Photocopy of certificate of eligibility/rating/license;
5. Photocopy of scholastic/ academic record such as but not limited to Transcript of Records (TOR) and
Diploma, including completion of graduate and post-graduate units/degrees, if available;
6. Photocopy of Certificate/s of Employment/Service Record/Special Order as proof of experience (if
applicable) and;
7. Photocopy of Certificate/s of Training/Seminar/Conferences as proof of training/seminars attended
declared in PDS.
