7. For Laboratory Technician II: Original copy of the Certificate of Employment with a list of duties and
responsibilities from previous/current relevant job experience; and
8. For promotion: certified photocopy of issuances, documents or any proof of your involvemement in
other activities and outstanding accomplishments which includes awards received from different
recognized and accredited international, national, regional, and/or provincial/municipal/city
organizations/institutions (1 copy).
